Pancreatic cancer has an 8% 5-year survival rate, and surgical resection remains the only chance at cure. Sarcopenia is an independent prognostic factor for increased morbidity associated with pancreaticoduodenectomy. Prehabilitation prior to surgery has been shown to maximize muscle mass and strength in order to improve post-op outcomes, and obesity may influence muscle development. Obesity induces a number of altered bodily functions and has been found to impact outcomes of exercise interventions. Non-obese individuals have been shown to experience greater responses to exercise compared to obese individuals, including increased muscle volume and knee and handgrip strengths. In this study participants performed 2 weeks of prescribed prehabilitation consisting of exercise and protein supplementation prior to undergoing pancreaticoduodenectomy. Linear regression and Wilcoxon signed-rank test were used to compare the change in lean mass and handgrip strength from baseline to preoperative assessment. Protein adherence and sex of the individual had significant interactions with handgrip strength and lean mass, respectively. These results should be reexamined with larger sample sizes to determine the true impact of obesity and potential confounding factors on the outcomes of prehabilitation intervention.
