Abstract: Extended-spectrum beta-lactamases (ESBLs) are the enzymes that are able to hydrolyze oxyimino-cephalosporins, that are inhibited by clavulanic acid, and reduce the activity of penicillins, monobactams and cephalosporins except cephamycins. ESBL-producing bacteria causing nosocomial infections continue to be a serious problem, whereas their incidence in community-acquired infections is increasing. Community-acquired infections due to ESBL-producing bacteria causing mortality and morbidity constitute an important problem worldwide including our country.Due to the increase of ESBL positivity in community-acquired infections there are many studies devoted to determine the precautions to prevent this increase and to investigate the risk factors. In this study our aim is to determine the risk factors for community-acquired infectious diseases due to ESBL-producing Escherichia coli and Klebsiella strains and their fecal colonization. A total number of 124 patients admitted to Başkent University Hospital during the period June 2006 to May 2008 with complaint of fever and who were not hospitalized in a month before the admission were included in the study. Clinical samples of the patients including either a feces or rectal swab culture, blood and urine cultures, and other samples required by the patient`s condition at the time of admission were evaluated for the presence of ESBL producing bacteria. The risk factors for the infections due to ESBL-producing Escherichia coli and Klebsiella strains, such as antibiotic usage within the last year and within the last three months, malignancy, prior hospitalization, invasive procedures, and the ESBL positivity in fecal culture, as well as the risk factors for their fecal colonization, such as gender, antibiotic usage within the last three months, and prior hospitalization were evaluated. Thirty four (27.4%) patients had microbiologically proven infection and 10 of them were due to ESBL-positive microorganism. The comparison of risk factors for the patients having an ESBL-positive infection and those having an ESBL-negative infection were found to be statistically insignificant: antibiotic usage within the last year (P.0.196), within the last three months (P.0.429), malignancy (P.0.095), invasive procedures (P.1.000), prior hospitalization (P. 0.127) and the ESBL positivity in fecal culture (P.0.061). Fecal colonization of ESBL-producing bacteria was found in 28 (22.6%) of the patients. The risk factors for these patients were not statistically significant either: antibiotic usage within the last year (P.0.086), invasive procedures (P.0.103). Fecal colonization of ESBL-producing bacteria were found to be statistically significant; antibiotic usage within the last three months (OR: 2.6, CI: 1.1-6.1; P.0.030) and prior hospitalization (OR: 3.7, CI: 1.2-11.2; P.0.017). Further studies are required for determining the risk factors for fecal colonization of and infections caused by ESBL-producing bacteria. There is a need of more comprehensive and wide scale studies for determining the risk factor for infections that are fairly important in community caused by the bacterias which produce ESBL and the risk factors for fecal colonization and for understanding the role of fecal colonization in setting.
