Abstract:
Özet: Araştırmamızın amacı, rezin esaslı yeni bir kök kanal dolgu materyali olan Epiphany-Resilon sisteminin biyolojik uyumluluğunun incelenmesi ve AH Plus, guta perka, Epiphany ve Resilon materyallerinin farklı kombinasyonlarının apikal sızdırmazlık yönünden değerlendirilmesidir. Biyolojik uyumluluk çalışmasında 36 adet, 9 aylık ve 200-220 gr ağırlığında olan Wistar Albino cinsi rat kullanıldı. Anestezi sonrası ratların sırtlarında dört adet subkütanöz cep hazırlandıktan sonra, her bir materyal (Resilon, guta perka, teflon tüp içerisine yerleştirilmiş Epiphany, boş teflon tüp) belirli bir cep içerisine implante edildi. Boş olan teflon tüpleri ve guta perka konlar kontrol grubu olarak kullanıldı. İmplantlar 1, 4 ve 8 haftalık süreler sonrası, etraflarında bulunan dokularla birlikte çıkarıldı. Histopatolojik değerlendirme sonucunda doku reaksiyonları derecelendirildi. İncelenen her zaman aralığındaki implant materyalleri arasında, doku reaksiyonu açısından bir farkın olmadığı belirlendi (p>0.05). Dördüncü haftadan itibaren doku reaksiyonu şiddetinin oldukça hafiflediği, 8 haftalık dönemde azalmaya devam ettiği gözlendi. Test edilen materyallerin tümü biyolojik uyumluluk yönünden yeterli bulundu. Araştırmada apikal sızıntının incelenmesi için 70 adet tek köklü diş, ProTaper dönen aletler ile crown-down tekniği ile prepare edildi. Örnekler rastgele olarak 4 adet çalışma grubuna (15'er diş) ve 2 adet kontrol grubuna (5'er diş) olmak üzere toplam 6 gruba bölündü ve şu şekilde dolduruldu: Grup 1, AH Plus + guta perka; Grup 2, AH Plus + Resilon; Grup 3, Epiphany + Resilon; Grup 4, Epiphany + guta perka. Tüm dişler 1 hafta süreyle nemli gazlı bez içerisinde kapalı kaplarda muhafaza edildi. Bir hafta sonra 10 +- 0,05 mm boyutunda hazırlanmış örnekler bilgisayarlı sıvı filtrasyon cihazına bağlanarak apikal sızıntıları incelendi. ANOVA testi ile gruplar arasında istatistiksel olarak farkların olduğu tespit edildi. Tukey HSD test metodu kullanılarak, Group 4 (Epiphany + gutta-percha)'ün apikal sızıntı değerinin istatistiksel olarak Group 2 (AH Plus + Resilon)'den daha düşük olduğu tespit edildi (p<0.05). Group 3 (Epiphany + Resilon) ile diğer çalışma grupları arasında istatistiksel bir fark görülmedi (p>0.05).
Abstract: The aim of this study was to evaluate the in vivo biocompatibility of the new resin-based Epiphany-Resilon root canal filling system and to assess the apical sealing ability with different pairings of AH Plus, gutta-percha, Epiphany and Resilon. Thirty-six Wistar Albino rats, 9 months old, weighing 200 to 220 g, were used to evaluate the biocompatibility. Four subcutaneous pockets were created in the back of rats, and each material (Resilon, gutta-percha, teflon tube containing Epiphany, empty teflon tube) were implanted into specific dorsal site. The empty teflon tubes and gutta-percha points were used as control. After 1, 4, and 8 weeks, the implants were removed with the surrounding tissues. The tissue reactions were graded after a histopathological examination. At each time period there was practically no difference in the reaction of all the implant materials (p>0.05). The intensity of the reaction had diminished by the 4-week observation period, and this reduction continued through the 8-week observation period. All the tested materials showed an acceptable biocompatibility. For the apical leakage study, 70 single rooted teeth were instrumented using ProTaper rotary files and crown-down technique. The roots were divided randomly into 6 groups (4 experimental groups of 15 roots each and 2 control group of 5 roots) and filled as follows: Group 1, AH Plus + gutta-percha; Group 2, AH Plus + Resilon; Group 3, Epiphany + Resilon; Group 4, Epiphany + gutta-percha. All 70 roots were stored in gauze dampened with sterile saline and enclosed in sealed tubes for 7 days to allow the sealer to set. One-week later, apical parts of roots of 10 +- 0,05 mm were attached to computerized fluid filtration meter and the apical microleakage values were determined. ANOVA revealed a significant difference among the groups. Multiple paired comparisons (Tukey test) showed that Group 4 (Epiphany + gutta-percha) had significantly lower apical leakage than Group 2 (AH Plus + Resilon) (p<0.05). Group 3 (Epiphany + Resilon) showed no significant difference from either Group 1 (AH Plus + gutta-percha) or from Group 2 and Group 4 (p>0.05).
