Özet
Some individuals with intellectual and developmental disabilities (IDD) emit challenging behaviors such as self-injury and aggression that pose a risk of harm to themselves or others. In an effort to maintain safety, restrictive procedures including physical restraint may be utilized. The implementation of these procedures carries inherent risk for both the staff that utilize them and the individuals to whom they are administered. One of the most important methods for reducing this risk is to ensure that staff who use physical restraint are properly trained; however, there is no consensus as to what constitutes appropriate training. The research in the area of crisis management training has focused on outcome measures such as decreased use of restraint, increased staff confidence, and reduction in injuries while not directly measuring change in trainee behavior. One potential method of measuring change in trainee behavior is by measuring both the accuracy and speed of the performance, a combination of outcomes that is referred to as fluency. Fluency may be increased through rate-building procedures consisting of brief, 20-s to 1-min., timed practices followed by feedback and differential reinforcement of high rates of responding (DRH). Purported benefits of this form of training include increased retention over time, decreased degradation of performance in the presence of distracting stimuli and increased performance in untrained but related skills of which the trained skill is a component. This study examined the effects of the addition of a rate building procedure to the training of a physical restraint procedure for new staff members who had previously demonstrated accurate implementation.
