Özet:
A simple, rapid and accurate method of High-Performance Liquid Chromatography (HPLC) with UV detector was used for determination of Carbamazepine (CBZ) which is the first choice anticonvulsant and mood-stabilizing drug and it is extensively metabolized in the liver by Cytochrome P3A4.
Tamarindus indica (tamarind), Mangifera indica (mango), Saccharum officinarum (sugarcane) are traditional juices which have been used for many medicinal aims, these traditional juices may interact with some drugs for example mango may cause inhibition of the CYP2C19 isoenzyme in the liver and this may lead to interaction with some drugs. Red bull is an energy-boosting beverage, caffeine is one of its major constituents and it has been reported to interact with some drugs on CYP1A2 isoenzyme.
The mobile phase was a mixture of 55% of water contains (1 ml triethylamine per 1 liter) and 45% acetonitrile, the pH was adjusted with phosphoric acid to be 6, BDS hypersil C18 column with particle size of 5 mum and dimensions of 150mmx4.6mm equipped with ultraviolet detection at 285 nm (UV-VIS Plus Detector) with flow rate of 1.0ml/min (Lc Pump Plus), using 15?l injection volume (auto-sampler plus) and 10°C auto-sampler temperature. Metronidazole benzoate was used as internal standard. The method was precise, and accurate Beverages were given in drinking water and pre-administrated to the rats before giving CBZ dose (10mg/kg). Plasma level of CBZ alone (group 1) was compared with plasma levels of CBZ with tamarind (group 2), mango (group 3), sugarcane (group 4), and red bull (group 5). Maximum plasma concentrations (Cmax) were 2222.78 ng/ml, 1006.3 ng/ml, 2090.42 ng/ml, 4446.31ng/ml, and 4523.67 ng/ml respectively for the five groups, group 2 (CBZ with tamarind) which was significantly decrease in the maximum concentration (p-value 0.05) and this may due to the low number of the rats.
The area under the curve (AUCINF) were 18048.1 ng*hr/ml, 16676.8 ng*hr/ml, 28136.61 ng*hr/ml, 35067.9 ng*hr/ml, and 34129.14 ng*hr/ml respectively, which were not significantly changed (p-value >0.05). The times required for reaching the peak of concentration (Tmax) were 0.35 hr, 1.6 hr, 1.5 hr, 1.7 hr, and 1.055 hr respectively, which appear to be significantly increased in comparison to the Tmax of the drug alone (p-value <0.05 of all of the beverages). In conclusion, we advise to be caution or to avoid the intake of the previous beverages since these beverages will delay the T max of the drug and will change the maximum concentration of CBZ in the plasma when pre-administrated, therefore dose adjustment during consumption of these beverages are needed.
