Purpose: The purpose of this pilot study is to compare the effects of whole body vibration (WBV) versus agility training on postural instability in patients with Parkinson's Disease (PD), Hoehn & Yahr stages I-IV, between the ages of 40--80 years old.
Methods: Eight individuals with PD aged 59--77, were randomly allocated to a whole-body vibration (WBV) group, n=4, and an agility group, n=4, whom underwent an activity protocol for 3x/week for 5 weeks including multi-sensory training, dual tasking, flexibility, and balance training. Postural control was assessed using the following outcomes measures: limits of stability (LOS) via Bertec Balance AdvantageRTM in reaction time, endpoint excursion and movement velocity, Fullerton Advanced Balance scale, and postural alignment via Dartfish 8.0. Significance level was set at .05.
Results: Significant results were found for the agility group in the LOS for backward (p=0.04) and composite reaction time (RT) (p=0.02). Additionally, significant results were found for the WBV group in the LOS for movement velocity (MVL) to the left (p=0.03) and for advances in postural alignment (p=0.02). Significant findings were found for the entire group in the LOS forward (p=0.005) and composite scores (p=0.005) for endpoint excursion (EPE).
Discussion: Positive changes were elicited by all subjects in > 50% in all measures. Furthermore, all higher functioning subjects (H&Y I & II) in both groups were able to make improvements in EPE, MVL and RT. Lower functioning subjects were able to make positive changes, but to a lesser degree.
Conclusion: Findings from this study suggest there is a relationship between EPE and MVL, as EPE seems to improve first and to a greater degree before MVL improves, but RT may require more time to change especially in the later stages of disease progression (H&Y III, IV).
