British government bodies have recently called for closer general practitioner (GP) involvement in the treatment and care of drug users. This thesis reports on a two part study. The first part is a survey of GP attitudes and treatment practices in respect of opiate users in three districts. The second part reports on short-term (3 months) outcome of treatment and long-term (approximately 2 to 3 years) outcome of treatment. The short-term study was conducted in three districts and two treatment modalities: 'single care' (by community drug teams (CDT) only), and 'shared care' (by combination of GP/psychiatrist and CDT). The long-term study was conducted only in the two treatment modalities. The first part of the study, using a postal questionnaire for GPs (n=270), found that, overall, GPs have positive attitudes towards opiate users. These attitudes differ between districts, with GPs in Central Manchester having more positive attitudes than GPs in the other two districts. Most GPs state that more CDTs and training would encourage treatment. The second part of the study, uses a structured questionnaire to interview opiate users (n=138). 72 subjects were followed up at Time 2 (3 months after initial interview) and 37 were followed up at Time 3 (2 years to 3 years after initial interview). Results showed that shared care produces better treatment outcomes than single care. Overall decreases in injecting behaviour and opiate use were found at follow-up. Subjective well-being of opiate users had increased at follow-up. District differences were also noted at Time 2. In general, treatment outcomes were better for opiate users in Central Manchester than in the other two districts. GP attitudes were found to have no effect on treatment outcomes. A Questionnaire has been devised to monitor treatment outcome for opiate users. Recommendations are made for more GP training in opiate use treatment and more shared care collaborations. It is also recommended that quality of life be taken into account in future research with opiate users.
