This thesis reports a study on 50 patients with verified supratentorial tumour. The electroencephalograms of each of them are compared with the CT scan. The thesis is divided into four Chapters. CHAPTER 1 Chapter 1 deals with the study of the EEC record of each patient and interpretation of its findings. The alpha rhythm of each patient was studied as regards its position, its reaction to eye opening and eye closure. The abnormal delta waves were studied and the polymorphic delta activity is regarded as a good localising sign. Theta activity and sharp waves were also studied. All the EEC abnormalities were marked on each of the four bipolar montages where they occurred and the maximal voltage of the delta wave was recorded in the average reference recording when they occurred. Twenty-nine patients showed phase reversals or focal delta activity and 46 patients showed one sided delta abnormality. CHAPTER 2 Chapter 2 describes the CT scan which was used for the investigation of these patients before the year 1976. At that time the CT tomographic cuts were made in six cuts 1.3 cm above the orbital meatal line and from the year 1976 and onward eight cuts were done routinely 1 cm apart. A trial was made to get the ideal level cut for each of the six or the eight routine cuts depending on getting exact levels on the lateral X-ray with ventriculography. Photographs of the so-called ideal level cuts were taken and line diagrams prepared. The mapping of the CT scan abnormality in each of the patients was done on these diagrams and revealed that 46 patients had unilateral lesions and 4 had midline abnormalities. CHAPTER 3 In Chapter 3 experiments were made on cadaver brains to identify the exact position of the electrodes in relation to the CT level cuts. Photographs were made of brains sliced at the appropriate levels and these were compared with the ideal CT cuts. The position of each electrode on the level cuts could be identified in this way. CHAPTER 4 Chapter 4 is concerned with the comparison of the CT abnormalities of a patient and the EEG abnormality. The data obtained from Chapters 1 and 2 were transferred to the ideal cuts with the electrode placement. It was shown that the EEG provided good localisation in 78.2% and lateralisation in a further 20% and negative results in 1.8%, with exclusion of cases with a midline tumour. As regards the alpha rhythm it is shown that the post-centrally placed tumour produced an effect on this rhythm in the way of diminished frequency and reduced reactivity to eye opening and eye closure. The frontally situated tumours did not produce any effect on the alpha rhythm. As regards the pathology, it is shown that the EEG of cases with astrocytoma and meningioma gave a high percentage of localisation but this was less with glioma. Eleven patients showed abnormal delta activity on one side and contralateral diffuse theta activity. After comparison with the CT scan it was shown that the theta activity was produced when there was a shift of the ventricular system to the side showing theta activity. It is concluded that the EEG is a reliable technique for localisation of supratentorial cerebral tumours in man.
