Background: Advances in supportive care have allowed treatment intensification and significantly improved survival from haematological malignancy. This has been at the expense of increased treatment related mortality, predominately due to infection. Following intensive cytotoxic treatment for haematological malignancy invasive fungal infection (IFI) is the leading cause of infectious mortality. These patients are particularly vulnerable to IFI due to iatrogenic factors, such as neutropenia, mucositis and immunosuppression. Genetically determined factors, such as mannan-binding lectin gene (MBL2) polymorphisms, may further impair immunity to fungi and predispose to the development of IFI. IFI is associated with a high mortality. Early treatment is critical in improving outcome from IFI, although diagnosis is often delayed, as conventional culture-based laboratory techniques are insensitive, samples can be difficult to obtain and are often obtained only late in the course of infection. Therefore prevention, including antifungal prophylaxis, and the development of molecular and serological early diagnostic techniques are important factors in attempting to reduce the incidence and mortality from IFI.
Summary: Our protocol for administrating itraconazole antifungal prophylaxis to SCT recipients was effective at achieving and maintaining adequate itraconazole levels. No link was established between MBL, L or H-ficolin deficiency and increased susceptibility to IFI in haemato-oncology patients. Finally, we demonstrated that panftingal LightCycler PCR may represent a significant advance in the early diagnosis of IFI, however, the Platelia Aspergillus failed to confirm the promising results reported in other studies.
