Immunoblotting work performed in this thesis provided a means of differentiation between members of the S.sanguis/S.gordonii/S.mitis/S.oralis group of viridans streptococci causing endocarditis. Previous work had identified endocarditis-specific protein bands for the second major group of viridans streptococci, the S.mutans group. This information identified high molecular weight endocarditis-specific bands for these groups but did not characterize these proteins. Therefore, one representative organism from each group, S.sobrinus MUCOB 263 and an S.oralis patienl isolate, was chosen for further molecular biological study. Genomic DNA libraries were produced lor each of these organisms, endocarditis-specific clones obtained by antibody-screening and subclones were generated by two different subcloning methods.DNA sequencing of these subclones yielded the complete S.sobrims DNA sequence for the 185 kDa endocarditis-specific band and an S.oralis endocarditis- specific antigenic region. The protein encoded by this S.sobrinus gene, (esa), was found to-share 99.2% identity with the S.sobrinus Spa A protein precursor and 70.1% identity with the S.mutans PAc protein precursor. These proteins play important roles in the process of bacterial attachment to teeth. The S.oralis endocarditis-specific region was highly homologous to a region conserved between the dental caries family of proteins. A synthetic peptide, representing this conserved thirty-nine amino acid region was produced and formed the basis of an indirect ELISA for the diagnosis of S.sobrinus/S.mutans/S.oralis endocarditis from patient sera. In an attempt to establish a function for the S.sobrinus Esa protein, this synthetic peptide was tested for binding activity to fibronectin. Fibronectin is found in a soluble form in the blood and as an insoluble form as part of the extracellular matrix involved in the injury repair mechanism. A fibronectin-like activity was discovered which suggested a novel mechanism by which the viridans streptococci can bind to heart valves, producing endocarditis.
