Patients with mental health diagnoses constitute one out of every eight U.S. emergency department visits nationally. Patients presenting to the emergency department with mental health diagnoses are often faced with long length of stay (LOS), ED boarding, lack of inpatient bed availability, and inadequate diagnosis-specific treatment. Targeted psychiatric care is often limited in emergency departments, and adequate resources are often not available. The burden on emergency services is high and psychiatric patients on holds awaiting transfers to appropriate psychiatric facilities or inpatient beds present a costly and complex problem for emergency departments and emergency providers.
Alternative ED psychiatric care models have been put into practice in recent years with promising outcomes. Such programs include designated psychiatric emergency services (PES) or crisis stabilization units (CSU) both within hospital emergency departments and in dedicated acute psychiatric care facilities, as well as the use of telepsychiatry. There is also increased focus on guidelines for medical clearance of psychiatric patients, as well as the development of standardized procedures. The variety of approaches to acute psychiatric patient care correlate to many factors including funding available at each facility, availability of trained staff, availability of beds, agreement on protocol to evaluate and treat acute psychiatric patients, and the general willingness of hospital ED's to make changes to their practice. Several approaches to emergency psychiatric services currently in practice are summarized here with the goal of working toward the development of a general guideline that can be employed to improve outcomes for psychiatric patients in the emergency setting.
