Cardiovascular disease is prevalent in Western society and includes a host of conditions which include heart failure, kidney failure and essential hypertension (HTN). Essential HTN in humans is often resistant to drug treatment leaving millions vulnerable to cardiovascular disease. In most clinical studies, renal nerve ablation reduced arterial pressure in resistant hypertensive patients and ameliorated other forms of cardiovascular disease. A few studies have challenged the effectiveness of renal denervation to treat resistant HTN, but many suggest that selected patient populations respond to this treatment. We propose that the effective results of renal denervation are due to the elimination of the renal sensory (afferents) nerves resulting in a reduction of the central sympathetic drive.
We sought to develop a means to ablate renal afferent nerves selectively which would serve to determine whether these sensory nerves are responsible at least in part for the development of HTN. Our lab developed an intrarenal approach to remove only the afferent nerves of the kidney in Sprague Dawley rats by depleting the neuropeptides in these nerves. We concluded that capsaicin or resiniferatoxin administration in the renal pelvis elicits a partial but selective renal sensory denervation. We also tested different concentrations and durations of exposure in the rat. Our preliminary data suggest that selective depletion of renal sensory neuropeptides will attenuate the development of experimental rat models of HTN. We next identified a large animal model (pig) to selectively deplete renal afferent nerves to simulate treatment in humans. Importantly, we showed that we could selectively deplete renal afferent nerves in the pig through a transurethral approach, but the denervation was less efficacious than in the rat. The differences in renal afferent nerve depletion between rat and pig suggested that we examine whether the localization of sensory nerves in the renal pelvis of these two species are different. If there are differences in the localization of these sensory fibers in the renal pelvis of rat and pig, this may suggest a physiological difference in the functionality of sensory fibers in the renal pelvis of these two species.
