Introduction: Globally, the nursing profession is known for caring. In the Kingdom of Swaziland, incidents of poor caring behaviors and unprofessionalism among practicing nurses has been increasingly reported; the public's trust in nurses is waning and the profession's reputation is tarnished by reports of breaches in confidentiality, physical and emotional abuse of patients and family, and widespread patient neglect. The purpose of this study was to develop a grounded theory illuminating the process by which nurses develop a practice that aligns with professional nursing values. The influence of emotional intelligence and compassion fatigue on nurses' caring behaviors was explored.
Methods: In the first component of this mixed-methods (Quant + Qual) study, a survey was administered to professional nurses in the country's 10 hospitals. The instruments included a demographics form and the Nursing Professional Values Scale -- Revised, the Assessing Emotions Scale, the Professional Quality of Life (ProQOL) scale, and the Caring Behaviors Inventory. Subsequently, respondents who volunteered for qualitative interviews ( n=54) were theoretically selected by their survey responses. Face-to-face grounded theory interviews were conducted until theoretical saturation was achieved and analyzed using methods described by Strauss & Corbin (1998).
Results: The data from the survey respondents (N=267) were analyzed using univariate and bivariate analyses, correlations, and multiple linear regressions. A majority of the respondents (65%) were female and relatively young (M=34 years) with less than a decade of nursing experience. Nurses' professional values (beta = .351, p = .000), emotional intelligence (beta = .143, p = .015) and compassion satisfaction (beta = .240, p = .000) explained nearly 42% of the variability in nurses' caring behaviors. Neither compassion satisfaction nor compassion fatigue moderated the relationship between nurses' professional values and caring behaviors; similarly, neither compassion satisfaction nor compassion fatigue moderated the relationship between emotional intelligence and caring behaviors. The nurse interviews (n=22) were transcribed and the qualitative analysis uncovered a variety of factors (modifiable and unmodifiable) that lead some nurses to have a practice based upon professional values and other nurses to have a practice that may be judged as uncaring or unprofessional. A core process of 'Sustaining Integrity' was identified and personal, situational, conditional, and contextual factors influencing that process were uncovered. There were three periods (prior to training, during training, and during practice) in which an individual may -- or may not -- acquire and master the requisite values for professional practice.
Conclusions: Addressing the unprofessional behaviors of nurses in Swaziland is crucial to improving the quality of care delivered to the Swazi people. Interventions to address the modifiable factors influencing the process of 'sustaining integrity' may include effective recruitment, curricular development, novice nurse orientation, and the establishment of policies and standards to guide nursing practice. Additional interventions may be designed to increase caring behaviors, ethical practices, and improved nurse-patient interactions. As a result, patients may seek care in a timely manner, accept patient education, and adhere to treatment regimens.
