Özet:
Congenital heart disease (CHD) is the most frequently occurring congenital disorder in newborns, with an often unknown etiology. CHD includes a variety of disorders generally defined as structural defects to the heart or coronary blood vessels. The mortality rate has dramatically decreased with medical advances, leading families to manage chronically ill children who sometimes have high caregiving demands. This leaves parents and the family unit at risk for emotional distress and poor adjustment, which may adversely influence child outcomes. Data from 381 participants was collected from the Herma Heart Developmental Follow-Up Clinic at the Children's Hospital of Wisconsin. As part of a secondary data analysis, this study utilized the Infant/Toddler Quality of Life Scale, the Family Impact Scale, and the Bayley Scales of Infant Development, Third Edition, at the child's six, 18, and 36-month appointments. The current study identified three stable trajectories of the impact of CHD on maternal emotions: high, medium, and lower functioning. Mothers in the high and medium classes tended to have high levels of education; while, mothers in the medium class also tended to have a child with a higher CHD disease burden. Children with a higher disease burden also tended to have poorer neurodevelopmental scores. The study also identified three trajectories of the impact of CHD on maternal time: high but stable, medium and improving, and lower but stable. Mothers in the improving trajectory tended to have children with the highest CHD disease burden; while, mothers in the lower trajectory tended to have children with the poorest neurodevelopmental functioning scores. Overall, only 5% or less of mothers were in the lower functioning trajectory classes, indicating that mothers of children with CHD are generally managing their emotions and the impact upon their time fairly well.
