Introduction: To address the clinical reality on the comorbidity of common mental disorders (i.e., depression and anxiety disorders), the theoretical preposition of common underlying psychopathological processes among these disorders, and the heavy disease burdens with their resource implications, transdiagnostic theory and treatment have been developed and investigated in the past decade. To date, there is a limited number of randomized controlled trials (RCTs) on transdiagnostic cognitive behavioural treatment with comorbid depression and anxiety, particularly in Chinese contexts. The present study aims at conducting a RCT with Chinese adults suffering from common mental disorders. The effectiveness of a locally developed transdiagnostic cognitive behavioural therapy for group (TCBT(G)) protocol was tested against the treatment-as-usual (TAU). A preliminary exploration into the potential treatment mechanisms was also conducted.
Methods: A total number of 103 participants with common mental disorders, i.e., depression and various types of anxiety disorders, were recruited from the community and randomized into two groups (TCBT(G), N = 54; TAU, N = 49). The participants were followed up for nine months post-treatment.
Results: The TCBT(G) showed promising outcomes with significant reduction in depression, anxiety and stress, as well as increase in work and social functioning compared to TAU with moderate to large effect sizes. Treatment gains were maintained or enhanced in the 9-month follow-up. TCBT(G) also significantly alleviated hypothesized underlying psychopathological processes such as dysfunctional schemas and catastrophizing cognitive distortions, while insights into these processes were increased. In addition, the TCBT(G) participants developed a more accepting attitude and tolerance towards unpleasant emotions, while displaying a willingness to try new coping strategies. However, the preliminary mediation analysis was unable to confirm these changes confidently as therapeutic mechanisms, possibly due to a small sample size and limited statistical power.
Discussion and conclusion: Compared to TAU, a locally developed protocol of a group TCBT(G) is found to be both more effective and cost-saving in treating Chinese adults with multiple mental disorders, namely, depression and anxiety disorders, in one single treatment regime. The therapeutic gains are maintained or even enhanced in a nine-month follow-up. Given such positive results, this local group TCBT(G) may be ready for broader application to the local community. Dissemination study is to be followed. Psychotherapy research in terms of exploration into the therapeutic mechanisms of a proven treatment protocol should be further pursued with more rigor.
