Özet: Yaşlanan nüfusun giderek artması, esas olarak ileri yaş hastalığı olan; tanısı, Önlenmesi ve tedavisi konusunda pek çok çalışmalar yapılan osteoporoza olan ilginin her geçen gün artmasına neden olmaktadır. Çalışmamızda, batı ülkelerinde geliştirilen osteoporoz teşhis ve tedavi etme kurallarına ülkemizde ne kadar uyulduğunu araştırdık. Çalışmaya; osteoporoz için tedavi alan, kemik mineral yoğunluğu (KMY) ölçümleri yapılmış 200 gönüllü kadın hasta dahil edildi. Hastalarla, osteoporoz için risk faktörleri ve daha önce yapılmış KMY ölçümleri açısından yüzyüze görüşüldü. Kaç kez KMY bakıldığı, istenme yaşı, ne nedenle istendiği, hangi hastaneden ve hangi branştan doktor tarafından istendiği sorgulandı. KMY raporlarının, omurga ölçümlerinde L2-L4/L1-L4, kalça ölçümlerinde femur boyun ve femur total ölçümlerinin T değerleri kaydedildi. Her rapor sonrasında hastaya önerilen tedavi, tedavi süresi, hastanın tedaviye devam edip etmediği, tedaviye devam etmediyse hangi nedenle tedavinin kesildiği sorgulandı. Hastalar 'National Osteoporosis Foundation' (NOF), 'The Osteoporosis Risk Assesment Instrument' (ORAI) ve Kanada osteoporoz kılavuzları temel alınarak değerlendirildi. Risk faktörü olmayan hastalarda, kılavuzlarda önerilmemesine rağmen, sadece menopozal dönemde olmaları nedeniyle KMY ölçümünün, sık olarak gereksiz yapıldığı görüldü (%55,5). Hastalardan en çok KMY ölçümü isteyen branş hekimi jinekologlardı (%30). Jinekoloji (%80), dahiliye (%48,8), fiziksel tıp ve rehabilitasyon hekimleri (%43,2) en çok menopoz nedeniyle ölçüm isterken, romatolog (%46,2) ve endokrinologlar (%45) en fazla risk faktörü varlığı nedeniyle ölçüm istemişlerdi. Kontrol amaçlı yapılan ölçümlerin, önerilenden daha kısa aralıklarla, gereksiz olarak tekrar yapıldığı saptandı. Birden çok KMY ölçümü yapılanlarda, ölçümler arasında, T değerleri arasında tutarsızlık (%28) olması dikkat çekiciydi. Antirezorbtif tedavi alması gerektiği halde tedavi almayan (%13) ve gereksiz antirezorbtif tedavi alan hastalar (%32) mevcuttu. Değerlendirilen 200 hasta için gereksiz yapılan KMY ölçümleri sonucunda 14,6 milyar TL ve gereksiz kullanılan anti rezorbtif ajanlar nedeniyle 83,4 milyar TL, toplamda 98 milyar TL harcama yapıldığı hesaplandı. Bu miktar T.C. Emekli Sandığından osteporoz için tedavi alan yaklaşık 72969 kişiye yansıtıldığında, KMY ölçümü için 53,3 trilyon TL ve antirezorbtif ilaçlar için 304,3 trilyon TL, toplamda 357,6 trilyon TL kadar gereksiz harcama yapıldığı sonucuna varıldı. Hastaların tedaviye uyumu genel olarak iyi idi. Doktor önerisi (%29,2) dışında, en fazla ilaç yan etkisi (%22,9) nedeniyle ilaç kesildiği gözlendi. Osteoporoz için anti resorptif ilaçlarla birlikte hastalara kalsiyum ve D vitamini önerilmediği saptandı.
Abstract: Osteoporosis is a growing disease that takes an enormous medical and economic toll on aging population. It is largely preventable due to the remarkable progress in the scientific understanding of its causes, diagnosis, and treatment. The objective of this study was to determine how the Turkish doctors manage osteoporosis especially according to economical point of view. Two hundred volunteer women, whose bone mineral densitometry (BMD) have been measured previously, were asked to join this study. Patients were questionnaired about risk factors of osteoporosis, number of BMD tests, the age of patient when the BMD was measured, the hospital where the BMD was done, the physician who ordered BMD and the recorded indication of BMD. T scores of BMD including L2-L4/L1-L4 spine, femur total and femur neck were filed. The patients were also asked for previous treatments for osteoporosis after BMD tests. The patients were evaluated based on NOF, ORAI, Canada osteoporosis guidelines. BMD tests had been repeated unnecessarily for the patients that do not have any risk factors, but only for being in menopausal period (55.5%). The most of the tests were ordered by the gynecologists (30%) when the patient was in menopausal period. Gynecologists (80%), internal medicine physicians (48.8%), and physiatrists (43.2%) usually wanted tests for the reason of being in menopause. On the other hand, rheumatologists (46.2%) and endocrinologists (45%) generally wanted BMD tests if the patients had the risk factors for secondary osteoporosis. The BMD tests made for routine control were applied unnecessarily, at more shorter frequency than suggested. There was a remarkable difference among the T scores of BMD tests of the same persons (28%). There were some patients who should take antiresorptive therapy, but they didn't (13%), contrary some patient took antiresorptive therapy, when they shouldn't (32%). It means that, some medical therapies were applied unnecessarily. For 200 patients 14.6 billion TL were wasted for those unnecessary BMD tests, and 83.4 billion TL for unuseful antiresorptive treatment. Another remarkable point we met was; this much a sum has spent just for 200 patients. However today, there are 72969 patients whose osteoporosis therapies' costs were supported by the Emekli Sandığı in Turkey. And, it means total amount of wasted money would reach to a sum of 357.6 trillion TL for 72969 patient. The patients compliance to therapy were almost excellent. Besides, the doctor's suggestions (29.2%) the main cause of breaking off therapy was side effect (22.9%). It was noticed that some doctors did not prescribe calcium and vitamin D within antiresorptive therapy and to those women who are peri or postmenopausal.
