The critically ill neonate with severe respiratory disease accounts for a significant portion of all neonatal transports. In the transport nursing discipline, the problem is a lack of evidence indicating the high-frequency ventilator is safe and effective to utilize during the transport of neonates suffering from significant respiratory disease, furthermore this study is essential to provide consensus to direct evidence-based policy. The purpose of this quantitative, ex-post facto, comparative study was to evaluate the efficacy and safety of utilizing high-frequency ventilators (TXP) during the transport of critically ill neonates to tertiary hospital destinations. Maslow's Hierarchy of Needs Theory provided a theoretical framework for this study. When this theory was applied to neonates suffering from significant respiratory disease, the first two levels provided a conceptual framework for the importance of achieving adequate oxygenation, ventilation and doing so safely during transport. This research study utilized a quasi-experimental, ex-post facto quantitative research design. The design utilized a causal-comparative method in which data were gathered and the variables were not manipulated due to the fact that they are naturally occurring. Evidence that the high-frequency ventilator is safe and effective to use during neonatal critical care transport is the main and most important conclusion of this study. The higher incidence of complications noted in the control group which was ventilated with the conventional mechanical ventilator provides sound evidence in favor or the safety of this mode of ventilation. The efficacy was indicated by the results indicating no difference in the ventilation and cardiopulmonary variables. The findings presented in this study indicates that the high-frequency ventilator is safe and effective to use during neonatal transport. The findings presented in this study provides evidence that the high-frequency ventilator is safe and effective to use during neonatal transport; however additional follow-up studies with much larger sample size is recommended.
