Introduction: Hookworm is a neglected tropical disease affecting approximately 500 million people of all ages. The primary approach to tackling hookworm recommended by the World Health Organization is mass drug administration of albendazole to children.
Objectives: This study sought to assess the prevalence of hookworm across age groups in Kintampo North Municipality (KNM) 7 years after Ghana's introduction of the national deworming program, to describe prevalence in children participating in a longitudinal study after its conclusion, and to investigate whether there are positive externalities to deworming these children for their close contacts.
Methods: 1,094 individuals, selected from 132 households where children were a part of previous longitudinal deworming study and from an additional 136 analogous households were tested for hookworm in four communities in KNM.
Results: The overall hookworm prevalence was found to be 23.5%. Adults continue to be significantly affected, with prevalence of 23.9% in individuals over 15 years of age. While a significant reduction in prevalence between 2007 and 2015 was found in the 6-10 and 11-20 age groups (from 57% to 26% and 59% to 27% respectively), adults benefitted from a much smaller burden reduction, to the extent that prevalence of hookworm in adults is equal to that of children. Prevalence among children who were dewormed biannually as a part of the longitudinal study was 6.1%, significantly lower than that of the source population (p <0.0001). However, the household contacts of these intensively dewormed children had no additional protection from infection.
Conclusion: Hookworm remains of public health importance in KNM. Given the persisting prevalence of hookworm in adults and the exclusive focus of deworming campaigns on school-aged children, these data raise concerns about the effectiveness of school-based deworming as the optimal tool for combatting hookworm. While biannual deworming lowers infection significantly, it failed to eliminate infection completely, and was not shown to confer protection to broader community.
