OBJECTIVE: Several barriers to insulin access and their potential negative health impacts for people with diabetes have been identified in the literature; however, these barriers are likely to be more problematic for people with insulin-dependent type 1 diabetes (T1D). In light of rising analog insulin prices and a lack of alternatives in the U.S., more research is needed to understand the experiences of Americans with T1D who cannot access insulin and how these individuals navigate such barriers. Toward this goal, we conducted a qualitative investigation to 1) describe the factors that affect insulin access among a cohort of U.S. adults with T1D, 2) characterize patients' behavioral responses to these barriers, and 3) understand the impact of these barriers and coping strategies across various aspects of patients' lives.
METHODS: A total of 21 adults who self-identified as having problems accessing insulin were recruited from online diabetes support groups using purposive maximum sampling variation and snowball sampling methodologies. Semi-structured phone interviews were conducted, and data collection and analysis were overlapping following a grounded-theory approach.
RESULTS: Participants conceptualized the experience or threat of being without insulin as a "life or death" emergency, which significantly influenced their responses to insulin access barriers. Participants described taking several creative, strategic, and agentive actions to maximize their existing supplies of insulin, obtain more insulin, and create long-term security against future interruptions in insulin access. These strategies were not always successful at improving insulin access, and often had collateral consequences for participants' physical and mental health, as well as their finances, careers, and relationships.
IMPLICATIONS: We propose that interruptions in insulin access among people with T1D can best be conceptualized along a spectrum of "insulin security." We propose several policy and practice changes to address this insecurity.
