In the United States more than 116 million people suffer from chronic pain, and 25 million chronic pain patients report, "significant pain-related activity limitations and diminished quality of life." Chronic pain affects more people than heart disease, cancer, and diabetes combined, and costs the nation up to $635 billion each year in medical treatment and lost productivity. Even though standards and guidelines have been developed for the care and management of the chronic pain patient, the most common reason for unrelieved pain is the failure of staff to routinely assess pain and provide pain relief. Chronic pain patients undergoing surgery are at risk for poorly uncontrolled post-operative pain, and if not treated, may lead to a delay in healing, stress, anxiety, increased hospital stays, and decreased patient satisfaction.
The author's current hospital process does not use a screening tool or process to identify, stratify, and coordinate delivery of care to opioid tolerant chronic pain patients in the academic medical center. The aim of this graduate project involves three distinct objectives that will improve early identification of pre-existing chronic pain patients' risk factors in the pre-operative setting. The primary objective is to complete a detailed literature review of the subject (chronic pain), and identify attributes, factors, and misconceptions that affect chronic pain patients in the acute care setting. The secondary objective is to complete a comprehensive review of validated chronic pain tools/questionnaires. The final objective is to develop a new electronic medical record (EMR) Chronic Pain Screening Questionnaire (CPSQ).
The specific questions asked during the CPSQ will be used to automatically trigger referral to a pain specialist, pharmacist, and psychologist, and/or pain management pharmacological and non-pharmacological treatments. This will assist chronic pain patient to have better pain control, improve function, and decrease opioid use. The tool can also be used to electronically collect, synthesize, and analyze chronic pain assessment, and identify areas that need change to increase safety and quality for patients. In addition, local and national pain experts will be interviewed to provide background on the subject and provide face validity for the development of the CPSQ. No patient records or individual identifiable information will be collected or included in the project.
