In the United States, exposure to childhood adversity is common; roughly 64% of children experience at least one moderate adverse event. An increased prevalence of disease related to cardiovascular, immune, and metabolic health is reported among those with a history of childhood adversity. This includes chronic obstructive pulmonary disease, ischemic heart disease, cancer, liver disorders, adult asthma, strokes, diabetes, as well as impairments in learning, behavior, and psychological functioning. A potential mechanism for this link is altered stress reactivity, though many inconsistencies in the literature remain. Acute stress activates a physiological response in the Hypothalamic-Pituitary-Adrenal (HPA) axis and the Sympathetic Nervous System (SNS), both of which exert their regulatory influence on the immune system; therefore, a dysregulated stress response may lead to maladaptive immune functioning. In five studies, we measured the HPA axis, SNS, and immune response to both singular and repeated acute psychosocial stress in healthy adults with a history of childhood adversity. We also tested basal activity of the HPA axis in this group, and examined whether cognitive appraisal plays a mediational role between childhood adversity and stress response. We found that those with a history of childhood adversity, particularly physical neglect, had an exaggerated SNS, HPA axis, and inflammatory gene expression response to stress, as well as less habituation to stressful stimuli during the repeated stress paradigms. We also found that that those with childhood adversity had blunted diurnal activity of the HPA axis, a precursor to several diseases; however, we did not find that cognitive appraisal helps explain altered stress responses. Across all studies, we found that childhood adversity was linked to altered reactivity of stress systems that regulate inflammation. This repeated over-activation of stress systems may be viewed as highly maladaptive, and as a potential marker for future disease risk in this population.
