Özet:
Metastatic breast cancer is a complex and incurable disease and is the most frequent cancer in women of all ages (American Cancer Society, 2016). Obesity is a global pandemic and is associated with an increased risk of breast cancer and poorer prognosis but only in postmenopausal women (Bianchini, Kaaks & Vainio, 2002: Keegan, Press, Tao, DeRouen, Kurian..., & Gomez, 2013). Epigenetic factors have been linked to breast cancer risk, however the data are conflicting based on age and menopausal status. This study was a secondary analysis of data from NHANES (1999-2010) that included the weighted sample of 6,878 records, representing 44,804,048 women. Logistic regression for weighted samples was utilized for inferential statistics. The predetermined study requirement of only including records that had complete data for all of the variables reduced the number of premenopausal women to three, which was insufficient for analysis. None of the variables related to the tests of hypotheses were statistically significant at the 95% level of significance therefore a correlation with the independent variables, visceral obesity, low-grade inflammation or the presence of both and breast cancer risk was not supported. Of clinical significance was the finding that two covariates were significantly associated with breast cancer. A woman with diabetes was 49% more likely to have a breast cancer diagnosis than a woman without diabetes (OR = 1.49, SE = 0.22; p = .007), and a woman with hypertension was 125% more likely to have a breast cancer diagnosis than a woman without hypertension (OR = 2.25, SE = 0.54; p = .001). These findings are hypothesis generating and may provide direction for studies including those aimed at identifying precursor biomarkers linked to diabetes or hypertension that may predict breast cancer risk in women of all ages and menopausal status.
