The routine semen analysis is of limited prognostic value, so tests of sperm function are being introduced to examine the functional integrity of sperm. The overall aim of the present study was therefore to establish a panel of laboratory tests and evaluate their potential clinical application. A clear disparity between agglutination tests and immunobead tests for detecting antisperm antibodies was demonstrated, the former exhibiting a high false-positive rate relative to the immunobead test due to non-immunological agglutinins. Whilst other tests using modified immunobeads showed excellent agreement with the IBT for IgA and IgG antibodies, the screening test (general SpermCheck assay) was poor at detecting IgM. It was demonstrated that the choice of cut-off value clearly affects the test result, suggesting standardization between laboratories is required. The penetration by sperm of human and bovine mucus in vitro was optimized after identifying the important influencing factors, notably incubation temperature and sperm concentration. Sperm washing, but not centrifugation alone, had a deleterious effect on the ability of sperm to penetrate mucus. Bovine mucus performs in a similar way to human mucus under the test conditions evaluated. The cryopreservation of aged, unfertilized oocytes by ultrarapid and slow methods was evaluated. It was demonstrated that oocytes preserved important biological characteristics after thawing and that there were clear effects of zona hardening. It was proposed that cryopreservation represents a successful method of oocyte storage for subsequent use in sperm function tests. The panel of tests was then tested in clinical applications. Sperm-mucus penetration was shown to be impaired by some types of antisperm antibody, but not others, suggesting the antigenic target rather than amount or type of antibody is important. In contrast to other studies, it was found that bovine mucus may be useful in assessing the effect of some antisperm antibodies. Pre-incubation with pentoxifylline of sperm from subfertile men did not improve mucus penetration, but oocyte penetration rates increased. Zona-binding of sperm showed an overall reduction, but individual results varied, suggesting some cases may benefit whilst in others the effect may be deleterious. In men referred for assisted fertilization procedures, the tests may be related to treatment by sub-zonal insemination, but intracytoplasmic sperm injection appears to bypass all sperm functions examined. The tests were shown to be useful in evaluating the efficacy of possible male contraceptive agents: the study found that sperm concentration fell after only one month of receiving testosterone injections, but sperm motility, morphology and ability to penetrate cervical mucus were only affected after a second month. A complete absence of mucus penetration was seen in all cases with sperm concentrations of <1.0 x 10e6/ml, suggesting this may be a more reliable target value for contraceptive purposes.
