Introduction: Chronic Fatigue Syndrome (CFS---an illness characterised by prolonged fatigue) is common, distressing and pervasive. Biological mechanisms cannot adequately explain the maintenance of symptoms and therefore research has turned to psychological models. Illness perceptions (e.g. conviction in a biological explanation) are thought to lead to behavioural avoidance, which in turn leads to physical deconditioning and the maintenance of disability. In chronic pain, another chronic disability, cognitive models of fear of physical movement and activity are proving useful in explaining the behavioural avoidance of pain patients, their disability and emotional distress. Aims: Phase 1 of this research aimed to develop and validate a measure of fear of physical movement and activity in a CFS population using an existing pain measure, the TSK (Kori et al, 1990). Phase 2 aimed to examine this fear in a CFS population, in relation to performance on a behavioural task, the perceived impact of fatigue and level of distress. Method: In phase 1, the modified TSK (M-TSK) and a subscale from the Illness Perception Questionnaire (Weinman et al., 1996) were administered to one hundred and four CFS participants. In phase 2, thirty-three participants completed a range of questionnaires including the M-TSK, a measure of the perceived impact of CFS and a measure of emotional distress. The participants were asked to ride an ergometer for as long as possible. This gave a measure of behavioural persistence. Results: The M-TSK was shown to predict behavioural persistence, perceived impact of CFS and emotional distress even after adjusting for gender, physical disability and level of symptom severity. Discussion: The results show that fear of physical movement and activity is important in understanding behaviour and disability in CFS patients. The wide theoretical and clinical implications of this are discussed. It is recommended that further research into this novel area is conducted.
