Özet:
A clinical and experimental study of hip excision arthroplasty has been made. The clinical results are based on an analysis of available case record material on dogs in which excision arthroplasty was performed and on the replies of a questionnaire sent out to dogs' owners. In addition a percentage of these cases revisited the hospital enabling detailed clinical examination and radiography to be conducted. The experimental results are reported on a series of sheep in which unilateral hip excision arthroplasty was performed. Following pre-mortem clinical examination these sheep were slaughtered at regular postoperative intervals. Radiography and histology were performed on the normal and excised hip joints of the experimental sheep to provide information on the formation of the resultant pseudarthrosis. In addition, tibial measurements were made on both the operated and non-operated limbs to detect any differences that were present. The inference of this study is that excision arthroplasty is a worthwhile operative technique for the treatment of conditions of the canine hip. There are however a number of postoperative features which have been considered abnormal. Some of these characters, such as limb shortening, muscle atrophy and limitation of joint mobility occur with such frequency that they should be considered to be common outcomes of excision arthroplasty. Continuous lameness is not a common sequel to excision arthroplasty and it seems that manifestations of lameness or particular locomotory difficulties result from strain of the fibrous and soft tissue elements of the pseedarthrosis. Large bony spurs left at the resection site can give rise to a less satisfactory postoperative result but small bony protruberances produce no long term difficulties. Remodelling of the resected site occurs postoperatively and such small spurs ultimately disappear. Weight bearing changes resulted in growth deformities in the non-operated limbs of the experimental sheep and the finding of patellar luxation in some of the re-examined dogs suggests that this could also a feature in the canine.
