With nearly seventy percent of Americans currently taking prescription medications, it is important to understand adherence trends and barriers in this vulnerable population. Research has looked at numerous factors that may influence medication adherence. One of the strongest correlations is the relationship between working alliance and medication adherence. This research study examined gender differences in medication adherence and working alliance among individuals diagnosed with either anxiety or depressive disorders. To assess these variables, the Medication Adherence Rating Scale (MARS) and the Scale to Assess Therapeutic Relationships in Community Mental Health Care (STAR) were administered to all participants. This study also explored the relationship between medication adherence and perceived stigma as measured by the Self-Stigma of Mental Illness Scale -- Short Form (SSMIS-SF). The findings suggest a significant and meaningful relationship between gender and medication adherence, as well as between gender and working alliance. In both measures, females rated their medication adherence and working alliance significantly higher than males. This study provided significant findings in support of the inverse correlation between perceived stigma and medication adherence. With a large population of individuals with mental illness not adhering to their medication treatment regimen, this data provides hope that there may be specific areas to target to improve outcomes. By improving working alliance between prescriber and patient, medication adherence will likely improve. Additionally, the significant correlation between stigma and medication adherence, suggests that improving working alliance between patients and prescribers, could reduce stigma and improve medication adherence. Limitations of the study, suggestions for future research, and clinical implications are discussed.
