Sexual minority women (SMW) disproportionately experience depression, suicidality, anxiety, and alcohol abuse relative to both the general population and their heterosexual, cisgender counterparts. This can be explained, at least in part, by the exposure of this population to sexual minority stress. Despite these well documented disparities, there have been few attempts to developing evidence-based psychosocial interventions targeted specifically to SMW. This project utilized qualitative research methods to inform the adaptation of an evidence-based psychosocial intervention that addresses cognitive, affective, and behavioral sexual minority stress pathways among SMW. Interviews were conducted with 19 SMW experiencing depression, suicidality, and alcohol abuse, as well as with 12 clinical experts who work with this population. A grounded theory approach was used to code and identify themes that could be used to adapt the Unified Protocol, a 10-week transdiagnostic, CBT-based therapy program, specifically for SMW. Interviews explored how minority stress processes impact the mental and behavioral health of SMW. Themes that emerged include the importance of facilitating insight into and skills for addressing SMW's exposure to adverse childhood experiences, sexual assault and intimate partner violence, and daily stress due to heterosexism, harassment, and discrimination. Participants also discussed how different aspects of identity, including gender expression, class, and race, moderate these experiences. Barriers to accessing culturally competent mental health services were also addressed. Ultimately, results from this project provide a framework for adapting a psychosocial intervention to simultaneously address the multiple adverse health outcomes SMW disproportionately face that are linked to minority stress processes.
